The Colorado State and Veterans Homes are located throughout the state, from urban Aurora to rural Homelake. The Office operates 686 beds that provide skilled nursing home care and 46 beds for domiciliary care (at the State Veterans Center at Homelake). The Office also provides adult day services for 11 clients at the Trinidad State Nursing Home. Please use the navigation menu to access information on the facilities.

The State Veterans Nursing Home at Fitzsimons in Aurora, Colorado opened on October 20, 2002. The facility has 180 operating beds, including a 21-bed Alzheimer's unit and a 24-bed rehabilitation unit. All facility beds are licensed for Medicare A.

The State Veterans Nursing Home in Florence, Colorado opened in January 1976. It has 100 operating beds, including a 14-bed special care/Alzheimer’s unit. The Florence facility was renamed as the Bruce McCandless State Veterans Nursing Home in October 2004 to honor a local Congressional Medal of Honor recipient.

The Colorado State Veterans Center in Homelake, Colorado, originally known as the Soldiers and Sailors Home, opened in 1890. The center consists of a nursing home and a domiciliary. A new 60-bed nursing home opened in 1991. The domiciliary has 46 beds.

The State Veterans Nursing Home in Rifle, Colorado opened on June 6, 1987. It has a total of 100 operating beds, including a 12-bed special care unit.

The Trinidad State Nursing Home in Trinidad, Colorado opened on April 1, 1957. It has 126 operating beds and an Adult Day Service Program that serves 11 clients.

The State Veterans Nursing Home in Walsenburg, Colorado opened on April 12, 1993. It has 120 licensed beds, of which 118 are operational. It has 100 beds on open units and 18 beds on a special care unit.
